I asked him to explain his actions.
I found this sentence in a dictionary. Can it be used in a situation where the male person did only one unfavorable thing? (I know it can when you talk about his usual behavior or a series of bad things he did.)

Yes, it can be used in that manner as well. In fact, saying "I asked him to explain his action." sounds slightly odd to me when used in that context.
