But I don't know in what case you use this phrase.
Could you please tell it to me with some examples??

Hi M-K-G,
To follow up on your promise means to do what you promised to do.

So for example,

"You told me that you'd cook dinner tonight. Are you not going to follow up on that?"
"Once you make a promise you have to follow through with it." (Spoken as advice to someone who's not very trustworthy.)
"I promised my parents that I'd visit them this weekend, so I feel like I have to follow up on that." (Said as an excuse when someone invites you to do something but you have to say no.)
