你叫什么（/什麼）名字﹖ - What is your name?/May I have your name?

A : 她是老師嗎 ﹖ - She is the teacher?
A : 她是老師吗（/嗎) ? - Is she a teacher?

You used the both of writing form; traditional Chinese character and simplified Chinese character. I think you should stick with either way.
And If your family name is Lee, you should put your family name first.
Something like 李瑪麗（/李玛丽）.
